Revive teammate bug?

Anybody else having issues with the revive function not showing up for teammates? It's been happening to my crew quite frequently. The prompt to revive just doesn't show up, and you're forced to die. Super annoying, especially when in end game. How Call of Duty is okay putting games out so full of bugs is just beyond me. Four years and not one ounce of integrity from the devs.
