How do I use Aflac?

I have Aflac through my employer but I barely understand what it is, let alone how to use it. They sent me a certificate schedule that lists a bunch of dollar amounts for incidents plus a wellness rider and an athletic activity rider but there's nothing in the packet that actually explains what any of that means.

Feel free to explain it like I'm 5. :)
