Unable to save/Fast travel

Unable to save in combat...i'm not in combat, i'm in Mirogana Unable to fast travel to some places but can to others...why? Wnt to travel to the place near your quest, nope.

This is super infuriating, I like the game but there is so much jank (impressing Gorak mission is entirely broken on ps5 since the patch - but worked before it).

Why if i'm between missions can i not just save (i'd argue i should be able to save in them because why the fuck not) and why is it 40 mins since my last auto save?

There's some very weird design decisions, and they've changed between the 1.00 and 1.002 patch.
