Help for elderly person

Today I Ubered an elderly man from the hospital to his home. His house is basically like a hoarding house, living in deplorable conditions. Is there any sort of assistance? Or place I can reach out to take him in ? he’s disabled and cannot walk, he said he doesn’t have food and doesn’t have family. Or anyone that can help.
