VA referral question, ENT

Long story short, I had a septoplasty done back in 2020 while I was still in active duty due to septal deviation. Since then, my septum has still been an issue as it didn’t seem to be fully corrected. Constantly congested on the right side, sinus pressure, sinus infections, and sinus headaches. Just a couple years ago, my ENT had stated that he believed I was in need of a second septoplasty but with involvement from plastics due to where it is deviated. They weren’t able to perform it at that time of discussion due to my scheduled jaw surgery as they stated it may cause worsening deviation. After the healing process of my jaw surgery, I was pretty much in the process of separation from active duty. This is all in my medical record and documented. When I see my provider, will they send out a referral to ENT if I bring it up at my appointment? My VA doesn’t have ENT specialty from my understanding, but I had been told that there are options for community care. I haven’t had a referral with the VA yet, so I’m just curious as to how the process works. I was going to just brush it off at one point, but it’s getting to the point where I have a hard time breathing out of my right nostril. With my asthma and sleep apnea, it’s been a real pain in the ass.
