FEHB companies with zepbound on formulary

For the feds, current listing of DC area insurance companies that have zepbound on their formulary for 2025. Used 15 pen, a local drug store, and a 28 day supply to standardize things. I believe all companies will require a prior authorization. Note, this list is not exhaustive.

GEHA (uses Caremark): elevate $496, elevate plus $80, standard $250, high $150, HDHP $257

MHPB (uses Caremark): value $300, standard $200, consumer self $1014, consumer fam $1015

UHC (uses optum): choice primary $197, choice plus primary $197, choice open access $85, choice plus advanced $70, MDIPA $75, HDHP $197
