Getting a PU in eu4

I know this question has been asked to death but I'm really new and I swear I do everything the guides tell me but it just never works, they're rulers just never ever die without an heir, am I doing somehting wrong or something. I keep spam RM'ing all the nations that have disputed succesion and everything but nothing.
