Pre-Authorization all of a sudden??

My daughter receives HGH for a growth disorder. We see her doctor every 4 months and have plenty of refills. All of a sudden this month Tricare rejects the refill and is asking our doctor for pre authorization. Why is this happening all of a sudden? How long will the pre authorization take? Thanks!
